Hi!

Wiki login is your ID on the wiki when you logged in to create the page.

The URL team is the part of the url after 2014 where your wiki page stands on the wiki. So if you're organizing an event in Sweden in the city of Uppsala and your team is SFD then the URL part to enter will be Sweden/Uppsala/SFD (as the full URL to your page should be wiki.softwarefreedomday.org/2014/Sweden/Uppsala/SFD
