[patch]: [doc]: escape some hash in the text

When you're sick of bugs, clean up the docs! This escapes some '#' characters so that "#define" and "#include" appear properly in the text. It also silences some Doxygen warnings (it was trying to interpret those words as links). Regards, Jeremy Henty
